I didn't make this, so rather than upload it, I felt it was more appropriate to give credit to Matt Smith and some links to the sources.

5e DM screen for DIY CD Case
And here is the one I made:

I like it because it's not so tall that I can't see the table or feel like I'm separated from the players, yet still have some privacy for certain dice rolls and notes.

5e DM screen for DIY CD Case
Updated
Screen has been updated with an alternate side panel that includes conditions. It can be used in place of either of the two side panels.
I built a DIY cd case DM screen for myself based on this guide (http://strangemagic.robertsongames.com/2011/04/new-diy-dm-screen.html) a few years back. I've been using it for a while with my B/X games using handwritten notes on the inside, but I decided to make an actual PDF that can be printed out for the rules in it.
How to Use
1. Take two clear CD cases
2. Remove the part that the CD would actually snap into, so that you've only got the two flat panels.
3. Tape both of the front panels together, as shown in the blog post above.
4. Print out the pdf. I recommend printing each panel on a separate page, rather than two-sided printing. The PDF is 8.5x11, but the actual screen itself is 10 inches wide. As long as you have a minimum of 10 inches wide paper and can print actual size, the actual paper size shouldn't matter too much.
5. Cut out the pages using scissors or an Xacto knife.
6. Use double-sided tape (or regular tape folded over) to affix the printouts to the player and DM sides of the screen however you prefer.
Where the Info Came From
Most of this came from the 5e Quick Reference Reddit thread (http://www.reddit.com/r/dndnext/comments/2bzxou/5e_quick_reference_sheet/). The Encounter XP tables came from the Basic DM PDF. The Monster XP by CR table came from looking at the monsters I have available in the free supplements that are out now (if you've got the monster manual, feel free to reply to this with the XP by CR for monsters of levels 11, 12, 14, 15, 18, 19, and 20).
Thanks!
That's all I've got for now! I haven't had a chance to print this out myself yet, and will be doing that tonight. Pictures of it in action are visible above, below the download link. I hope this screen is useful for you!
