Do weapons and armor lose plusses when they go from plane to plane like they did in 2e?

Depends on the plane. It's a bit different from 2e, so I highly recommend that you check out the Manual of the Planes.

After a quick glance at my book, I don't see a centralized list, so you'll probably need to look at each plane.

In general, no. At least I don't see any indication that they do.
