Monsters with Combat Advantage bonus damage

If a monster deals extra damage when it has combat advantage, does it still follow the same rules as Sneak Attack?

For instance, if the monster makes more than one attack, do both attacks deal the extra damage? Or, like Sneak Attack, the bonus damage is only dealt on one attack per round?

Unless the Combat Advantage entry says otherwise, the monster applies it to all attacks and damage that qualifies.

However, most monsters with Combat Advantage abilities rely on it for their threat, without which they tend to be weak.
