I do 3 things related to skills in games that I run.
1) Increase all (2 + int mod) classes to (4 + int mod). This is due mainly to the fact that being able to put points into things like Knowledge: Local and Speak Language tend to be pretty important in my games. So far, people that didn't know that fact beforehand pick up on it pretty quick once play starts.
2) The following are class skills for every character, regardless of class: Craft, Handle Animal, Knowledge: Local, Profession, Ride, Swim. Refered to as Every-Man Skills. Mainly represents things you picked up in the time before a character was a might warrior, wizard or whatever.
3) Give access to the Fast Learner feat. Fast Learner is a general feat, only selectable at character creation, that gives +1 skill point per level.
