I am not sure how you accomplish this since horses, while Large, only have a 5' reach. Also, a horse can only make one attack if it moves more than 5' since it is restriced by the Full-Attack option on multiple attacks.

OTOH, you could hit your opponent at 10' with the lance and then Overrun (PHB p.139) the opponent with the horse. Just don't do this against an opponent with a Guisarme or a Halberd. They can make Trip attacks against you to unhorse you. Especially if they have the Hold the Line feat from Sword and Fist.
