I would say Roy's movement is artistic license, one of the many times OotS ignores the RAW for a better comic.
 


Also, remember that Roy is not 5 feet wide by 5 feet thick; he doesn't fill a standard 'square' of grid space by himself, he has to move around a bit in his 'square' to strike out at his full 5-feet of reach outside that 'square'. A D&D character is always assumed to be moving a bit in their space during a fight, after all.

Yep. I've also seen it in the Warcraft RPG. The Gladiator/Blademaster prestige class.
 


Eh, originated in the first 3.0 rules supplement, Sword & Fist: A Guidebook to Fighters and Monks, with the Master Samurai prestige class.
 

Please for goodness sake if your going to use this make sure you can only take 5' steps limited to your total movement. Not a step every time you cleave. This may seem like common sense but you know what they say about common sense. :)

I say this because in my favorite campaign we had a dwarven berserker type guy who was allowed to take a 5' step after every kill. This allowed him to move around an entire clearing in about 2 rounds killing 30 or 40 goblin archers. Amusing to watch but probably not balanced. :D
