Just picked it up about an hour ago, seems pretty good from a quick look through. One thing I noticed quickly was the Sadako/Samara Ring type ghost. It even leaks water and has an attack that contorts a targets face.

Check out the Unrisen. Pet Cemetery apparently scared one of the designers as a kid.

It's an 11th level template (elite soldier) for a humanoid, beast or magical beast.

The worms are treated as a disease.

I notice quite a few monsters eat healing surges and/or prevent people from using them.

(Which reminds me, monster power idea: eat a healing surge of a target, then do extra damage equal to healing surge's value worth on next attack).
