1. Conditions last until they are countered or until the duration runs out. I would rule that shifting into a snake or something similar counters the prone condition.

2. I would say it works underwater normally with no disadvantage and that attacks against it have no effect.

3. It depends on the size of the stone. A improvised ranged weapon inflicts 1d4 damage, but the DM can rule that a bigger rock does more damage.

4. To keep it simple, I'd rule lack of sleep the same as lack of food (Basic Rules, page 66).

5. Yes, though I would say the attack has disadvantage.

1) I don't recall any restrictions against casting while prone (after all your just lying on the ground) so I think they can do that, especially sense it's a class ability not a spell. As for the snake being prone I could see it ruled either snakes are always lying on the ground they can't BE prone; or being prone represents being knocked off balance/tripped up (in some cases) so it has the same effects as being prone and the snake is simply tangled up with itself, imagine tied in knots.
2) I would say a spiritual weapon works under water just like any others, though the caster either has to be above water when casting it or be able to breathe water sense you can't chant underwater. I'd give it disadvantage like I would any other weapon. As far as the spiritual weapon being attacked I'm not sure; you could probably do something like a sunder attempt (attacking a weapon/piece of equipment) and if it succeeds it counts as it being stunned for a round or something but I don't think weapons could destroy it. I don't know what the last question is asking.
3) That one is extremely variable you'd have to know the situation to tell 'cause it depends on terrain, strength of character distance to person he's trying to throw it at (farther it is the lighter the stone would have to be).
4) I think it be something like a con check every night or half night you go without sleep with a starting DC of ~10 and increasing by 2-3 every half night or 5 every night based on experiences I've had (I stay up WAY to late basically every night, frequently not sleeping at all.)
5)I'd say yes though distance is increased by one weapon range or something so if it's in normal range it counts as long range and has disadvantage, if it's at long range it can't be done. Alternatively it could be all attacks automatically at disadvantage and each foot of water counts as ~3 feet of air or something.

In 5e there are very few rules, especially in regards to environment, so there might be official rules pertaining to one of these situations, MAYBE, the vast majority of the time you just have to make your own calls/judgments.
