Obscuring Mist questions

I dont see any errata for these so:

1) The range is listed as 30', but the area of effect is described as "...centered on character..." I assume the second is correct?

2) A creature 5' away has one-half concealment. Is this measured from square center to square center? Ie, does 5' mean you are in the square next to your target? If it is, then people in melee have one-half concealment. If a 5' distance means that you are two squares from your opponent though, then I'm not sure how often it would come up. I suppose it would apply to reach weapons.

The spell creates an immobile, 30-foot radius cloud of mist, centred on the caster's location. The "range" entry just means that it's possible to make the cloud bigger via the Enlarge Spell feat.
