D&D 3E/3.5 Off Balance in 3.5?

There used to be a "off balance" state in 3.0 didn't there? I'm not seeing it in the 3.5 SRD and am curious to know if it was taken out (and why) or if I'm just going mad.

If I recall correctly, it made you flat footed but without all the uncanny people piping up.

That says you're "flat footed" when balancing, which means that Uncanny Dodge lets you keep your Dex.

Didn't there used to be an "off balance" effect that was immune to Uncanny Dodge? I guess I should have kept those ratty old books :\

I have this in my notes (for the OGL game I'm working on):
A person who is off balance loses their Dex bonus and grants a +2 bonus to those attacking.
Is that in 3.5 anywhere? If not, where was it in 3.0?

It wasn't a specific state, like "flat-footed", "dazed", "panicked", etc. You just lost your Dex bonus and people got a +2 to hit you.
