Once I had the players go to arrest a group of knights whom they knew to be behind the recent attacks of a green dragon and her dragon kin army. Once they reached the chapter house and busted in they found a cable of wizards who summoned a couple of low level demons to tangle with. Eventually after fighting their way through the place. One of them asks where all the knights are? And I let them know that knight is a title and that most of the knights in the Order of the Silver Star are wizards.

And the people who lived in the game world all their lives didn't know that Knight was a title anyone could have? I really hate when GMs pull that sort of nonesense.
 

It wasn't nonsense. All of the character's were from other lands, only one was noble born and this particular group of knights was fairly recently made. The whole thing was one of the dukes decided that his army would be more powerful if he spent all his resources training and recruiting wizards instead of warriors. It all ended with civil war and the age of magical revolution.
 

Ravenloft, the original adventure, has been a favorite for my group for many years. I've converted it to 3e, 3.5, and to 4e.

The main villain is placed randomly based on the fortunes read by the Vistani Madame Eva.

When we ran this in 3.5, the party entered the castle and decided to immediately go upstairs, as they had never had a chance to see that part of the castle. In the second room they ran into the main villain. One of the players was incredulous that it could be the BBEG. He wildly commented, "that can't be Strahd, it's too early to be Strahd." Needless to say, some of them ran away after summarily getting their butts handed to them. Some of them didn't make it out at all.

To this day when something looks out of place they use the phrase, "That can't be Strahd, it's too early to be Strahd."
