No, if the types are listed as "and" you need to meet both. If they were listed as "or" you could use either, but I don't know of any that are listed as such.

Most lower power demons and devils are this way. For demons it goes good or cold iron, good, good and cold iron, good and cold iron and epic, as you progress up the power scale in the MM. for devils it is the same way just substituting silver for cold iron.

I hope Miko will beat up Xykon (and I think she can do it, she's pretty high leveled, Reddles and Xykon have wasted quite some spells and she only lost once so far ... against melee types. Not against puny spellcasters).

Then reddles will run away again to the next gate with Xykon around his neck.

Its pretty clear to me from past strips that Miko is unaware that the throne gem is the gate itself ("if she had leaned little more..."). She should also be unaware of the existance of the Martyrs (it was a "family secret"). However, the ghosts are clearly those of saphire paladins, and they're fighting the lich and Redcloak, so she would really have to redefine the term "lawful stupid" not to recognize their purpose. IMO. ;)

Miko has proven herself to be the benchmark in Lawful/Stupid.

So much a benchmark that questions are raised. Note that
1) Miko asks for "signs" alot.
2) Whenever she asks for one, she sees an answer in something.
3) She follows the "answer" to its logical extremes.
4) Miko is high level.

The above imply that she has interpreted many, many "signs" before, enough that, if the "signs" were not real, random chance would have stripped her paladinhood long, long before the OotS showed up.

Was Miko set up? If so, by whom? She certainly used to have a direct line to the gods... should we really blame her if a divine strength being started hacking the system (or if her gods decided to have her hang herself out to dry)?

I wonder if Mr Burlew is taking a leaf out of 'The Deed of Paksenarrion'? In which it is explained that sometimes evil deities like to give paladins a semblance of their powers.

Xykon isn't an Evil Outsider, though. Miko's contact with the Creature in Darkness wasn't exactly peaceful, and her encounter with Sabine was anything but. So, I don't think she meets the prerequisites to become a Blackguard.

But is "The Tangles" an "Evil Outsider"?

Miko decides that the Saphire Gate was what corrupted their ruler and the Saphire Guard. Clearly what is needed is to release "The Tangles" and join it to destroy it. Of course in Miko's opinion only a person of true purity, like herself, could succeed.

So much so, that it makes one wonder how she handles paladinhood with a Wisdom score apparantly lower than Belkar's.

As for being set up, Miko was set up by Rich.

On the contrary, her wisdom score is peerless. If her "sign requesting" behaviour in the OotS is not atypical, her ability to discern what is a sign and interpret it correctly (read to its logical extreme without losing paladinhood) must have been astounding in the years before she met the OotS. Not astounding enough to determine that either the gods chose to abandon her (although if they did so with ulterior motives, a la Paksenarrion, her behaviour is as they wish...) or some other being of equivalent power has hijacked the god-phone, but astounding regardless.
