It is an artifact from an earlier edition of the game, after all...

It's possible to have an item that does that in 3.5, even by the RAW, they just don't spell it out.

It could be any Belt item for example (like the Monk's Belt, thus useless to most characters, and the OOTS, as well as being the cheapest "belt" core MI). It has a 5% chance by the RAW of being cursed. On the curse type table, there is a 15% chance of a "Drawback" to the item. On the "Drawback" table there is a 3% chance (30-32) of "Character's Gender Changes". Thus it's fully possible even by 3.5 RAW to have a magic belt that's only function (to almost any character) is to change characters from one gender to another.

Now, as for being as hard to undo as the old 1e/2e Girdle of Femininity/Masculinity, that's not quite there (removing the belt and a Remove Curse should do the trick).
