He was the last priest of orcus when orcus was dead and all memory of him had been scoured from the multiverse.

Could you possibly be thinking of Return to the Tomb of Horrors? That product was written so that its background intersected with that of Dead Gods, one of which was noting that in the past, the living Acererak had had something to do (this was deliberately ambiguous) with a being known as "Tenebrous" - which was an interesting anachronism right there, since Orcus only took that title as a contemporary moniker, not back in the past when Acererak was alive.

After playing through to the point when you reach the Tomb, our DM wanted to play, so I took over, (and then we stopped playing) so I read the Return in preparation, I have clearer memories of that.

Might he have been mentioned in The Great Modron March? That was the prelude to Dead Gods, I believe.

I believe he was also used in Paizo's Savage Tide adventure path as commander of Orcus' armies. I don't recall any photos of him there, however.
