Planning sessions

First off, i posed this under general rpg forums. If this is considered spamming forgive me.I posted this here because i am running a pathfinder game, though its a broad question as well.

Like i said, im running a pathfinder game now. I've been playing/DMing rpgs for about 11 years now. My question to you is... How do you plan your sessions? And im not necessarily looking at this for pathfinder, i run a multitude of games such as L5R, 4th, HERO, GURPS , D6, etc..

A more specific inquiry would be , has anyone come across any sort of sheets that are formatted for a consistent look. I've been looking to see how other adventures are written, and most of them vary in how they are written . Or how the adventure is presented. Myself , i basically take a sheet of paper, and write down how I would like the story to go, and then note relevant skill DCs and monsters/pages.

There really isn't any rhyme or reason to it, and i was wondering if anyone has a more consistent format they use that is easy to understand.

TLDR: Does anyone have a session/encounter planning sheet?

from start to finish, it should the intro/adventure hook to the session, then it should be information to help you with the groups initial decision, after that just the story, any skill checks, and with any encounters, have some quick stats for the monsters all on the encounter page, and at the end of all the pages have the entire list of all the monsters from all the adventures fully statted out. If I can find one of my recent ones I'll upload it for you.
 


Planning is good; but I have also found that sometimes, and I say SOMETIMES, a bit off the cuff can be fun. I have put the most innocence thing into the game, say a rumor, that literally was just a rumor for back ground flavor, and my PCs were off and running with that. It made for some interesting times. Like I said, on occasion its nice, and also, I don't like and I try not to, railroad, an adventure. But too each their own.
