In two ways:

You get one plot point, can spend one per session

1. Player introduces a twist. One example given is stating that a monster currently in play is actually a long lost ally polymorphed into a bestial form. At the group's option, the player to the right of spending player introduces a complication related to the twist.

2. No permanent DM. When a player spends a point, he or she becomes the DM.
