Quick Survey: Most important spell stats

I'm toying around with a project of mine, and I was wondering...

What are the stats you find most important for a 3E D&D Spell? Say one was designing a compact spell sheet, that has room for a few items, plus a line or two for general text. What would be the 5 (or so) most important parts of a spell you'd want clear labels for on a character's spell sheet? Below is an early draft of what I'm talking about. What are the items you'd want to most see on the sheet?

Apart from what you have, Save DC for that spell cast by that character, whether spell resistance applies, and what page of what book the full description can be found on.
 


Page/book reference. Very good idea.

Things like save DC would be found on the character's main character sheet. I don't think damage would be a good field to put in, since its not too generic: there are alot of spells that don't do any damage. A field for SR though would be good, though again that applies only for hostile spells. Hmmmm....

Casting time is crucial. There's a big difference between 1 action and 1 full round, not to mention those that require minutes or more...
