The feat says you gain an extra attack at your highest base attack bonus. Does this mean literally your BAB (and thus DEX and other modifiers don't apply), of does it mean your highest ranged attack bonus?

You get a bonus attack at your highest base attack bonus, and then apply all modifiers as normal. So effectively, it's at your highest ranged attack bonus. Don't forget to take the -2 to all ranged attacks in the round though.
