I see a few things:
You have medium armor proficiency, but not light armor proficiency on your feat list.
You also don't have an armor bonus to AC (making it 18)
Weapon proficiency feats don't exist in the houseruled system (see the section on proficiencies).
You have 12 wound points and as many VP as you would normally have HP using decimal average of all dice except 1st level (31 vp total)
I personally think urban tracking should use gather information. I know survival is a generalised skill that covers urban environments, but it just doesn't seem to work when a combination of other skills makes more sense. It's also difficult to explain how you learned how to use regular tracking in an urban environment. Drop or explain how you got survival and track on a city-world, and use gather info for urban tracking instead (I don't recall if it has track as a prerequisite. If it does, you don't need it. anymore unless you can think of a reason for having it.)
