This may seem terribly simple, but there are freeware programs to test the load on a server...why wasn't anything tested...again?

Oh, it probably was tested, but the people didn't test properly: wrong procedure, wrong input, or just the wrong test load (basing the number of people trying to hit the server based on an inaccurate estimate.
