In my campaign every night the party is effected by "madness" basically, something happens to their character. It can be physical or mental or something else. It is usually bad, and always permanent. These are high lv characters so don't hold back. Here are some examples:
Lose a random limb: The character loses a limb, no pain or bleeding involved.
Gain a random limb: The character gains a limb in a random place, like a hand on his head.
Expand: the characters size doubles
Shrink: The characters size halfs
Fang: the character grows fangs
Shadow rage: If another somebody steps in the characters shadow, the character will fly into a rage, and try to kill the offender. This lasts for ten minutes.

Anything you can come up with would be helpful.
I need to have around a hundred of these madness effects, so I am pretty desperate.

Blindness: Character goes blind
Deafness: Character goes deaf
Gender Change: Character's gender changes
Infatuation: Character falls madly in love with first living thing they see
Odd Infatuation: Character falls intensely and romantically in love with the first non-living (including undead) thing they see
 


thanks, I will be using those in my campaign.

As for madness, it is really just a name for random horrible permanent effects. The only reason I am calling it madness is because the campaign is set to take place in the "Outhouse of Madness Themepark". The things submitted by Moe Ronalds are perfect examples of what I am looking for. Thanks very much by the way.

Just In case you are a fellow invader zim fanatic who is wondering. This in fact a themepark based on the "Outhouse of Madness",which is the replacement for the "Holding Pen Of Pain" in from the episode Hobo 13.
 

Body change: characters change into random bodies but keep all stats and abilities.

Characters change into random monster bodies or take the form of monsters.

Stat change: have characters move 1d4 of a stat into another category (example: move 1d4 strength to wisdom)

skin texture changes: character grows scales or skin becomes soft and sticky or things of that nature.

Character has to shout to be heard or even a wisper is now deafening.

Character blacks out into random fits of rage or depression at inopportune times.

Character gains multiple personalities (designed by the DM but that have to be played out by the character)

Character gains fire affinity or cold affinity, thus making the opposite a weakness.

Character has uncontrolled hair growth while sleeping. Wake up looking like Bigfoot every morning.

Characters take on a single animal like trait (flippers, tiger eyes, lobster claws, hooved feet, Moose hornes, etc...)

Character speaks a different language daily (randomly generated.)

Albinoism

Character grows a devil's tail.

Characters joints bend backwards.

Character gains third eye that allows "special vision" but clouds all other senses.

Ok I'll try.

Extra head: original head withers and dies, possibly becoming undead. and obnoxious.

Synthesthesia(sp?): The character hears color, sees sound, smells touch or any other permutation. It's not really that simplistic, but the gist should work for an offbeat game like the one you're running.

E.T. : Ever see the Cheech and Chong movies?

Arms change to prehensile vines, feet grow roots. Character needs a pot?

Character now has to sing everything (in rhyme).

That's all you're getting for free. ;)

Wierd game, but I've run some strange ones in my time.
 

Character gains a stutter.
Characters skin is covered with a sticky secretion, Strength check against DC (?) to unstick.
Character loses all or part of their skeleton, dexterity reduced by ?.
Character becomes ethereal.
Character gains amnesia.
Character gains fiendish qualities
Character gains celestial qualities
Character gains the ability to eat anything, and must eat unusual substances to gain sustinense (metal, wood, stone, etc etc)
Character takes on a ghastly appearance (-? to Charisma).
Character must eat twice as much food as normal.
Character becomes a carrier of disease (gains immunity to disease and can spread the disease via touch (this applies to touching anyone or anything)).
Character gains a phobia (fearful of something, swords? ;) ).
Character gains a phelia (loves something to absurdity... the worst being necrophelia).
Characters skin becomes harshly sensitive to clothing and armor, thus must remain naked at all times (this would not include jewelry or adornments).
When casting spells with verbal components, Character must yell them to make the spell work (this includes command words for items and using scrolls).
Character gains a serious odorous problem, those within 5' of him must make a fortitude save against a DC of 12 or become nausiated for 1 round.
Characters hair grows to 5x its length and becomes prehensile (treat as a 3rd limb with a strength of one half the characters).
Characters blood becomes a corosive acid (those who wound the character must make a reflex save of DC ? or take 1d4(or whatever) damage).


Ok, I'll stop for now... but those are some ideas I was able to cook up.
 


For RPing purposes,

Character must end each spoken sentence with the word _______.(whispering or shouting optional!) I personally like the word Cucumber.
 

The character periodically and randomly belches fire (anything from a burning hands effect to fireball..)

Whenever the player rolls a ___ the character falls asleep (pick a number or range of numbers on a d20)

THe character's shadow becomes sentient and talks constantly, espcially about secrets and embarrassing moments from the character's life. The constant jabbering is incredibly annoying (concentration check to cast spells, almost impossible to move silently, etc...)

Whenever the player rolls a ___ their character's alignment becomes the exact opposite and the character has a psychotic/paranoid episode.
