Ring of Gaxx

Where has the Ring of Gaxx appeared? I know it was in Eldritch Wizardry, the first edition DMG, the Book of Artifacts, and the Epic Level Handbook. Where else might it be found?

I am aware that it was also in the Baldur's Gate II computer game.

A d20 set into a ring? That's the Ring of Attaxx.

It's an easy mistake to make.

It was described in Eldritch Wizardry as a ring with a gem (8 sided?) in a mount so that it could have different faces rotated to allow particular powers to be used - but the faces couldn't be marked and rotated silently at odd times, so each day you never knew what the first use was going to be. I always imagined it to be an octagonal prism on a spindle rather than some magically rotating d8 though.

Interesting word choices, Plane Sailing!: I've toyed for years with making the Ring of Gaxx a master controller of Ioun Stones, and making synergies between the powers of the two---the ring amplifies/expands the powers of ioun stones, among other things.
