I've been tinkering with starting a personal website, so I recently pulled together a complete list of my credits. I'd prefer to post a link, but since my site is not yet up, I'll just post the whole thing here. It's complete so far as I can remember.

1) Reply with your screen name and the name under which you've been published.

2) List your publications! When possible, put a link to a place where folks can buy 'em. Since one of the goals here is to support EN World, for print books I'd suggest linking to its entry at the EN World RPG Shop; for PDFs at RPGNow, add &SRC=EnWorld to the end of the link.

That was such a fun module to DM. My players thought they were smart and smash all the boats at the waterfall so they couldn't be followed. Then realized they still needed one, oh wait there is one up there on the beash. hahahahaha

(It has become a reoccuring tale of ill planning for our group)
