Hello,

Take your PHB 3.5 look at the cover page but reverse the book to have the title in the bottom and in the red gems you'll see a guy with a camera who take a picture. They had probably made the cover from a real book and they take a picture in high resolution to print the book.

Yes they did. Sorta. Artist Henry Higgenbotham do fake books, and then photographies them.

I remember having seen pictures of the whole fake books on the WotC site.
