Could be, but at one point he claims that it's in the MM. In that same passage, he sort of implicitly admits that there is no template, since he suggests using the stats for a rat swarm (minus the climb speed and the filth fever presumably).

It seems to me that the obvious way to make jermlaine dangerous is to tack on levels of cleric. They've got +6 Wisdom adjustments.

There is a sort of "swarm" template called the mob template for 12 large or 48 small or medium critters to get together. It is in the DMG II, p. 59.

Suddenly 48 commoners get dangerous, yo!

But the Jermlaine is tiny. Not big enough to make a mob, yet not detailed in the MM as a swarm. Pity.

I think a swarm template would be a good idea, though.
 


The question is still good though because some PCs, e.g. high-level barbarians - have DR. But they're missing obvious answers like allowing the Jermlaine to Power Attack or have Weapon Finesse, or even (shock, horror) give them class levels.

I think the swarm template is from Worlds Largest Dungeon. I could be confusing it with mob though.
