Save Penalties, how to get them?

I'm working on a cohort that reduces enemy saves for a level 20 one-shot.

So far, I've got hexblade 5, ravager 8, blackguard 3

Hexblade 4 alternate companion causes penalty on saves. (Can be dispelled)
Blackguard 3 aura of despair causes -2 penalty on saves in a 10ft radius.
Ravager 8 aura of fear causes penalty on saves in a radius. 3/day.
Hexblade curse causes penalty on saves (but requires a failed save to work)

Shaken condition is -2 on saves.
Sickened condition is -2 on saves.

Any ideas on how to inflict shaken and sicken condition realistically on enemies? Anything that requires a save will probably not work so well.

(I left out some of the details with the Hexblade and Ravager stuff to comply with copyrights)

Intimidating Strike in PHBII lets you make a regular melee attack as a standard action and if you hit you may roll in an intimidate check (or something along those lines).
I think brutal strike in PHBII lets you cause an opponent to be sickened if you nail them with a bludgeoning weapons but they get a fortitude save.

Sounds like a cool character idea.
