I heard somebody mention in passing a “paladin in hell” module and the very name stirred my creative juices. Here's a cool idea for an NPC for use in a Planescape setting. Please note that this Pally requires some pretty serious rule-bending for the sake of flavor.
This NPC can be used to help good NPC's or can be the BBGG for an evil party. BTW, I wanted this guy to be able to pull of 4-person TPK's at CR 17, but I'm not convinced he can do it. My lvl-17 planar champion might could take him with one hand tied behind his back. Any further suggestions? He just seems sort of weak, even after I really tried to pump him up.
Sebastian Andovar the Condemned
It is said that pit fiends fear little, and balors fear nothing. But a legend, passed down throughout hundreds of years, chills even the haughtiest Tanar'ri or Baatezu. Stories tell of a fallen paladin who travels the lower planes, bringing death and destruction in his wake. Few have seen him. The lower planes are vast; some, infinite. Yet not even the deepest level of the Abyss goes without tales of his passing.
Sebastian Andovar began as a great paladin of noble heart. He was a champion for all that was good on the material plane. He was, basically, your iconic paladin. It was not long, however, before the forces of evil found his fatal flaw: the blind love he held for his bride, Miranda.
A cabal of disparate evil forces came together to plot against Sebastian. They conspired to have Sebastian come into possession of a powerful artifact; a blade that condemned all slain by it to an eternity petitioning in the lower planes. Before he was able to destroy the blade, the fiends acted out the second part of their plan. Through deceit and trickery, Sebastian was convinced that Miranda had been unfaithful to him. In a fit of rage, he slew her with the cursed blade. Sebastian's enemies gleefully revealed their plot, bringing suicidal despair to Sebastian, who took his own life with the blade.
But the deities of the upper planes were not done with Sebastian, and would not let him off so easily. As punishment for his heinous act, Sebastian was again granted life, and condemned to spend eternity searching the lower planes for Miranda. Furthermore, he is granted a strange form of immortality, so that his anguish can never be ended by the peace of death.
And so he searches. A LG paladin, spending centuries in a universe where he can feel the oppressive evil around him, is finally driven mad. Declaring war on all the powers of the lower planes, he dedicates himself to their destruction, becoming CG. He wages a one-man war against evil in a universe where good cannot prevail. He is the ultimate heroic tragedy; his life is grief, guilt and wrath.
His mount is Hazakar, a fiendish pegasus who is blindly loyal to the fallen paladin so unlike himself. As further humiliation, Sebastian is followed on all his travels by the pitiful and pathetic-looking twin fiends Matoiotes (or Futility, as some call him) and Kopetos (also known simply as Despair), who appear as dirty, malnourished, twisted-looking waifs. All they do is follow and watch. They do not act or speak. They stand in stony silence and watch Sebastian fight, eat and sleep. Sebastian does not speak to them, return their stares or even acknowledge their existence. When he comes to town, however, he provides money for their room and board, and feeds them from his own rations when they travel. Sebastian accepts them as just a small part of the punishment he feels he deserves.
Hazakar is never left alone with good characters (or anyone, for that matter) lest he should act outside of Sebastian's own good tendencies. Matoiotes and Kopetos never engage in combat; they will simply blink out of danger and continue staring at Sebastian.
Throughout the centuries, Sebastian has acquired a veritable treasury of magic items that increase his power and infamy.

He may not be able to know whether you're evil or not, but knows if you're lying. This ability is extraordinary, and is always active.
Spell-like abilities:
Summon Nightwalker 1/day: Sebastian abhors this ability and uses it only if he must. The nightwalker follows Sebastian's instructions unfailingly. He may banish it at will, and usually does so as soon as possible.
Darkness 3/day
Condemnation 1/week: As dismissal (caster level 20), but target is instead banished to a heinously torturous imprisonment on Carceri; Sebastian will not use this ability on anything other than evil outsiders.
Rebuke Undead:
As a level 17 paladin would turn undead. Again, Sebastian would rather just cut them down, but he will use this ability if he is forced to.
Frightful Presence:
His very appearance is dread and fearsome to the denizens of the lower planes. He can employ frightful presence once a day (spell-like ability), as the dragon ability, will save DC 30.
Righteous Might:
Sebastian is not entirely abandoned by his former patrons. As the sole agent of righteous wrath in the lower planes, Sebastian has been divinely endowed with SR 20 and DR 30/+2.

This pitch-black armor is made from adamantite, an adamantine alloy from the most radioactive levels of the abyss. It has a natural +3 enhancement that functions just as adamantine armor does. This armor also has 100% fortification, making him immune to critical hits and sneak attacks.
Sword of Life Stealing:
Two negative levels on target upon successful critical, 1d6 temp. hp's for 24 hours. Has a +2 enhancement bonus.
Ring of protection +4
Shield of the Dragon:
This large pitch-black adamantite shield functions as a large steel shield, but with an additional, non-magical +2 armor enhancement. It is shaped like a fierce dragon head, and can use the breath weapon of any adult chromatic dragon every 1d4+1 rounds.
