Simple PC tracker

I'm jumoing back in to the GM's chair after a three yaer absence and I am looking for a quick Excel based PC tracker for ntoes during a session. I was wondering if anyone here could make a recommendation.

My primary needs are having a quick reference card for basic PC stats that I don't want to ask them for through out the game like, spot, listen, sense motive, AC..... Somthing similar to the campaign log from back in the late 70s.

I could create one but I wanted to see if there where some more creative ideas out there. I have seen some that border on the insane with the amount of data they collect.

Look near the bottom for: "Initiative Cards (Fantasy)" you can cut them out or leave them as a sheet. What I do is leave them as a sheet and have the players fill one 'card' for their character and then just put two pages back to back and slip them in a page protector.
