We do many things depending on what people want. Last few times though a different DM has started a completely new campaign.

Yes. I've done all of them. Typically, I use it to make a new game (possibly in a new setting) but it depends on my mood and my Players moods

I'm just keen to know what other Dungeon Masters do in this situation.

I've never had a TPK yet (I musn't be trying hard enough!), but if I did, it would basically involve letting the players roll up a new party who would have the same goal as the deceased party, if feasible.

If it wasn't feasible, or the players didn't want to, I'd let them start with a new party on a new adventure path...
 


Depends on the players and how keenly interested both I and they were interested in continuing this game.

If the PCs aren't interested I MIGHT continue or try something else.

If I'm not interested (rare but it does happen) I'll try something else.

If we're all tired I say "We break for a month and come back."
 

I first dance my DM Jig o' Death while taunting them with a couple of Who's Your Daddy's.

Then another DM steps in, as we alternate between 4 DM's, each with their own Campaign.
Makes for some pretty long breaks in DMing.

I've had a few... I generally do one of the first 2 options you mentioned. I like running my Greyhawk setting, so I stick to the same. However, I usually change the locale within the setting if we opt not to continue on with the same campaign with new characters.
 


Or New Group, Same Setting, Different Goals, Different Adventures Followed By Derived From The Old Adventures, And Most Important Of All, The Looting Of The Old Characters Gear From Either Their Corpses Or Its New Owners.

Ever since I was introduced to the idea of a new party exploring the same dungeon as old characters and having the opportunity to loot them, I've been attached to it. It smells continuity mixed with the essence of the dungeon adventure.

Of course, then there is the idea of filler game with new characters in the same world, where you purposefully kill them so that when you return to the main event, the heroes can loot their temporary characters and confront their killers, hehehe... Either way.
