Or: a new pair of arms sprouts out of your shoulders. And start to strangle you, trying to tear themselves free with the rest of the second You that the Wish created inside your body........
 

Thanx for the replies guys. As for the Noble Djinni, I saw that the word "wish" was in italics in the MM but I hadn't known that it meant "look up "Wish spell"".

The italics generally means that it is a game specific magical effect: either a spell, or a magical item. It is, as far as I am aware, entirely consistent throughout the entire documentation of the game (SRD, PHB, DMG, MM....).

It's cool. In general, though, VERY few things (to my knowledge) are left that vague. It would be incredibly uncommon for a classic D&D monster or adventure or whatever to have a passage that simply says, "The PCs can get something awesome here" without saying anything else. Simply saying "He can grant three wishes" without specifying what that means would be unheard of in the rules-lawyer world of D&D.

There was a spell in Savage Species that gave you extra arms. As a DM, I would have the Wish duplicate that spell effect.
