If I use Spell Immunity (Clr 4) to become immune to flame strike, am I immune to the 5th level cleric version as well as the 4th level druid version? And what if a druid used heighten spell to make it a 5th level spell?

Yes, you would be immune to Flame Strike in both situations. It is on the 4th level druid list, so you may choose it for this spell, but you are choosing the spell, not the list. So you are still immune even if a cleric casts it, or even if a druid heightens it. It is still Flame Strike.
 

This also applies to metamagiced versions of the base spell. For instance, in your example you would also be immune to an Empowered, Heightened, Maximized, or Intensified Flame Strike.
