But then you've still got the problem that someone who stands up and drinks a potion provokes two AoOs from someone with Combat Reflexes - presumably because he leaves two gaps in his defences.

Should someone whose entire round consists of one big gap and no defences provoke as many AoOs as the opponent has available? It doesn't really make sense that someone who isn't held can possibly provoke more AoOs than someone who is... :)

Heh, the logical conclusion to that is to allow someone with Combat Reflexes to take all his AOOs against a helpless foe if he chooses to :eek:. Leaving none for anyone else, of course.

No.

Yes.
How about when grappled, and can't get the material component associated with the spell of the same name?
Yes. However, you will need to make a Concentration roll on account of it being hard to focus while someone is twisting your arm. DC 20+spell level, IIRC.
