There's sort of a mutually-assured-destruction truce among my gaming circles over Holy Word and Blasphemy. If the players don't try any tricked-out Holy Word shenanigans, then the DMs avoid doing the same with Blasphemy.

Has anyone else "soft banned" spells or abilities like this?

Not really, but I'm very unlikely to use lots of confusion effects- I just plain don't care for them, and at higher levels they're useless anyway (hi mind blank!)

Instant death, disintegration, etc- yeah, I'm down with all of that stuff. But as a dm, I never fudge my saves against those things when the pcs use them, as they can't fudge against me. It's only fair.
