Stirge touch attack?

Under most circumstances, it would be quite stupid of the stirge to take the last 5' that way, because it would involve stopping 5' away. Either way it suffers an attack before it gets to attempt one, and if it just closes and sucks up the AoO it gets to do so earlier. Plus, stirges often come in groups, and most characters can make only one AoO per round.

Most of them. The stirge is Tiny, has no reach, does not threaten an area, and provokes an AoO when it attacks.

mmm... intelligent shadow stirges stepping out of the ground... so wrong... so very very wrong... :D

Keep in mind that if the stirge only moves 5' there's no aoo.

Incorrect. A Tiny or smaller creature provokes an AoO, not for leaving the threatened space (although it may do that as well), but for entering the space the larger character is in. This AoO is provoked regardless of the distance of the move prior to entering the square. (See the discussion of Size and Face in the DMG.)

What discussion of Size and Face in the DMG? I know of no such discussion in the DMG, and would appreciate a reference. Do you mean the one in the PHB (p. 130)? That one just says that sufficiently small creatures moving into other creature's spaces provoke AoO's "as normal," begging the question of just which "normal" rule is being referenced. I always assumed it referred to the fact that to enter another creature's space you almost by definition have to leave a threatened space (unless, of course, the creature whose space you are entering does not have any reach itself).
