Are there existing rules for attaching a sword to a pole to create a polarm, or a special modification I can put on my weapon?

Strapping a sword to a pole ? I think you can safely call something like that an Improvised Weapon, using a glaive as the basis for damage and reach, though I'd be tempted to take the damage down a notch from the glaive.

The end result would be Reach 10', damage d10 (or d8 if you kick it down a notch), crit on 20, x2.

That goes without saying, doesn't it?
No...not really.

Another thing that needs to be said is that a sword can't just be juryrigged onto a pole, and thought of as a combat worthy polearm. Swords have tangs...glaives do not. Therefore, it would have no solid contact with the polearm itself, and no reinforcement. The sword would HAVE to get reforged, and reformed into a polearm head...

...otherwise it will snap on first hard contact.

Yeah, I was thinking along the lines of a naginata. Maybe a hole with threads in the bottom of hilt of the sword, and a protrusion with matching threads on the end of a pole? If such were the case, i don't think the -4 would apply.

Well, it depends. Are you going through the long and annoying process of making Craft Checks to craft the item? If not, then it's not a crafted item, it's an improvised weapon. A well-thought-out improvised weapon is still an improvised weapon. A weapon made without any sort of crafting check isn't a crafted weapon.

It all depends on your DM, but there has to be a line between making a weapon and slapping two weapons together.
