Who was the original inventor of this terrible creature?

I can find references to it as far back as 2nd Edition versions, but I see several anecdotal references from Googled sites of people saying "my friend so-and-so created it" (never the same person).

So where did it start?

And also, is such a creature doable using standard 3.5E rules (that is, any 3.5E rules WOTC has published itself)?

Using just the MM, no. There might be another way to apply a lich-like template to a magical beast (especially one that can't do it on its own), though.

Unaugmented tarrasques are still nifty though.
