The Peasant hero who becomes a king, the simple savage who slaughters thousands and speaks in short sentences, the Ogre mercenary who saved the Princess, the Dragon Slayer and even your own version of the Black Knight in Shining Armour...

Not really a hero, but with strength and martial prowess a huge brute of a man named Sir Artus de Courseilles earned the nickname "The Horse-Breaker." He wields a great maul, and had a habit of slamming it into the chest of oncoming cavalry chargers, sending the whole mess of horse and rider crumpling to the ground.

Sir Emory Destain is known as the Knight of Four Swords, and more of a hero. He has four blades strapped to the side of his warhorse, named Glory, Honor, Duty, and Faith. He only draws Glory at tourneys, and Honor when called to duel. Legend says he draws Duty in times of war, and so far as anyone knows, he's never drawn Faith. Speculation on what might cause Sir Emory to draw his final blade is plentiful.
 

The heroes and warlords.

Zoldac: NG Half-elven Ftr6/Mystic4 - A rising star in the Technology God's clergy. Likes counterspelling enemy buffs before wading into melee behind his more resilient companions. Raised alternately between his parents; he crusades for technological and societal progress while advocating protection of nature. Trying to move beyond the shadow cast by his illustrious parents - one a crusading human adventurer, the other a famous elven diplomat. Uses 2-bladed sword in melee and handcrafted musket in emergencies.

Stanley the Butcher: CE Human Brb 14 - Possibly the most depraved treasure hunter to ever break down a dungeon door in centuries. Devout worshipper of Erythnul; inhumanly strong (not to mention short-tempered), Stanley is a one-man murder machine. Is known for slaying anything that gets in his way - including two party members. Can operate alone easily; rarely needs healing due to DR and magic items. A real sod.

I can post the stats on this guy if needed. He's a fighter, albeit a strange one from my game world.

Blackheart
Blackheart is an orc, but has become much more than that. Now residing in Undercity, he rules this hive of villany and trechary with a much fairer hand than any human that has ruled before. Blackheart, once known as Grok The Savage, came to Undercity as a mere mercenary. He was much smarter than orcs usually, and just as tough. He was the pride of his mercenary captain, and second in command. He would have been the leader except no one trusts an orc, and an especially smart one even less. Undercity was then ruled by a local ganglord who was mercilous in pitting his second in commands against each other. Undercity was known then for slavery and kidnappings of the citizens above ground. Blackheart had always felt that slavery was the worst thing a creature could do to another, and one of the main reasons for leaving his home tribe many years ago. He led a massive slave revolt nearly a decade ago in Undercity to overthrow the ganglord. He was only known as Grok before, but became known as Blackheart during the revolt. It is said that that title was what the ganglord called him when Grok threw him from the battlements of his own underground castle. As he stood their with the ganglord over his head the man cursed him as a "Blackhearted beast," before Grok threw him to his doom. The name stuck and Blackheart has ruled the Undercity better than any man. He has led it's forces against Illithiad sorcerors, dark elf armies, and even a dragon some years past. Now he is honored, at least below the city, as a fair and just ruler. It is said, "Though you might not find a fair deal in the city above, that there is always justice for those who seek the rule of Blackheart."
 

Well, in a campaign I played in a year or so ago with one of my groups, I had a human Barbarian named Ichi who wielded a magical katana. He started off just as a run-of-the-mill barbarian, but, over the course of the campaign, he became a legend. This was because A. he, as well as the rest of the group, happened to get into some freakishly hairy situations and then somehow find a way out of them, B. most of the situations they got into threated the entire world, and 3. none of the other characters in the group wanted anyone else to know that they were even there- therefore, all of the glory got pushed to Ichi.

Even though I'm no longer playing with that group, it's sort of a running joke in my current group (since I've shared with them the stories of Ichi being eaten by a shark, or fighting an ex-god, or fighting an ancient two-headed slaad-lord thing) that if there's ever any fairy tales or legends of a powerful warrior, they're about Ichi.

Ichi was, I'd say, my favorite character I ever played- and, as a result, I've even written a few compositions (at least one of which was written as an assignment for school) detailing his backstory and/or inner thoughts. If anyone's interested, send me an email and I'd be happy to share them.
 

First, ancient heroes, in order of when they did their heroics...

1. Aranion, the son of the first High King in 'time immemorial'. He saw his dad cut down by the BBEG. In a glorious, single cut, he killed the BBEG and completely turned the tides of war.

2. Twin brothers Karadir and Tiradir, a few thousand years later (they're 'ancient history' at the time that the campaign takes place). They started with nothing more than a single fief, and built a great empire. Both were peerless swordsmen and riders, and won many battles by slaying the enemy general and causing the enemy troops to rout. Ultimately, they both died to the same BBEG that killed Aranion's father, however.

3. Elondir, second son of Karadir. He never did anything terribly notable as a warrior, but as a commander, he proved to be one of the greatest in the war against the BBEG.

4. Arandir, Elondir's younger brother. He's the ultimate hero; the third son of the king, he never stood to inherit much in the way of lands. When his father's capital fell, he and Elondir fled through the mountains, and got separated. Arandir found an ancient tomb, and in it, the sword of Aranion; the blade that 'killed' the BBEG the first time. He and his brother were reunited, and they gathered an army, with which they swept through the kingdom and retook their father's lands. In the end, Arandir defeated the BBEG in personal combat. After that, he went on to found his own kingdom in the east.

And now, 'modern-day' famous warriors in no particular order;

1. Hieral, king of Dornlund. He personally defeated the king of Ennoria (a powerful nation) on the battlefield, and in the process ended the bloody war. He's rather old by 'campaign present time', but he could still take a man's head off with his sword if threatened.

2. Jonir Innan. He was once a pirate, who rose through the ranks to captain. His sword-hand is what won him his position, but it was his wits that kept him alive. When the High King that had ruled during most of Jon's career died, his son--a great admirer of the pirate's exploits--pardoned him and made his ship--the Blue Scourge--part of the royal navy. After that, he fought countless naval battles against southrons, most of which were victories.

3. Galethorn, son of Calloran, lord of Greyhill. He's only just beginning his career as a hero, but as far away as the PCs are, his exploits are at least known of. First, he saved the Dornic village of Goldberm from certain destruction by goblins. Then, he led a small group of warriors into the very home of the goblin horde and back. The last great thing he's been rumored of doing is holding a bridge from 8000 goblins for more than an hour with only 1200 men. They say he's at least six and a half feet tall, and uses a dwarf-forged bastard sword that cuts through iron. More impressive is the rumor that he's only twenty, if even that old.

4. Delethorn, son of Elondar, heir to Elondale. Delethorn is one of the PCs; he's a stalwart knight who does most of his fighting on foot with an ancestral greatsword called Elondir's Fury. So far, he's known quite well in the eastern kingdoms for unhorsing Prince Kieran of Dornlund, and to a lesser extent, in the west for his deeds while in the besieged city of Fort Tirad.

5. Eben, giant, good-natured oaf. The eight-foot tall Eben is the last of his race and one of the PCs; with his axe or his bare fists, he makes for a memorable warrior; in Elondale, he won a wrestling match at the tourney--against four strong men. In Fort Tirad, he's made himself well known as the Lord Admiral's personal cook and bodyguard.
 

Tongoth - An orc vampire of old. He blackmailed the great city of Kharanov, threatening to kill them all unless the King's attendant wizard to craft him an artifact blade. After the sword was made for him and he set about murdering thw people anyway, he stumbled upon the underground lair of a Gray Dragon and met his end. Okay he's more of an evil hero.

Jarne - A simple human farm boy who caught the eye of the great Tourmaline wyrm Mogystinaeas. The dragon apprenticed the boy under him and oversaw his growth into the great warrior of his time. However, as old age crept up on him the gods offered him a place in their ranks, if he could slay the dragon. Jarne asked Mogystinaeas to grant him immortality before he collapsed of old age, but the dragon refused, according to the draconic belief that mortals should stay that way. The god of crafts made Jarne a great sword, Dracomor, and he slew the dragon and took his place among the gods, eventually becoming the deity of war.

In my campaign, dragons and gods are opposing forces, thus, Jarne is a hero to those who worship gods.
 

Lemme try this in that cool format everyone else is using:

1. Ichi the Undead Slayer- a man spoken of in legend and stories of ancient heroes, Ichi is known for his countless heroic deeds, which include saving the city of Daggerdale and the surrounding country by defeating entire armies of the undead god Kiaransalee, destroying the iron fortress of the warlord Imperagon, and singlehandedly defeating the blasphemous dragon Ashardalon in the Bastion of Unrborn Souls on the positive energy plane. Due to Ichi's fame, his once-small hometown of Uthmyr has become a booming city with countless visitors all coming to visit the legend's home and to have a drink in the city's largest establishment, the tavern "Ichi's place". Ichi is said by some to be a tall, muscular man with long, dark brown hair, wearing a large, black amulet with the image of an ancient black dragon. He wields a glowing magical katana which, according to legend, houses the soul of his deceased mentor, and the blade is said to desintegrate any undead it touches with a mere touch. When in battle, his image is a sight to behold- some say he turns into a thirty-foot tall beast that breathes fire and flings spells from each appendage, flying with a mere thought and destroying mountains in a single swipe. In any case, not a mortal has been known to gaze upon Ichi while in a rage without fleeing in terror. Ichi has been known to be loud, raucous, and is said to drink anyone under the table when he comes to a new tavern- and he does so often. Ichi is said to be a frequent traveler of the planes, although the majority of his past (and even whether or not Ichi is his real name) is shrouded in mystery.
 

These are great! Galethorn I love the way you have created a mythic dynasty, and UltimaGabe 'The Legend of Ichi' is just perfect worldbuilding (please share more).

Anyway some of my own Fighting Legends have been

Kala Human Barbarian/Wardancer. Kala was raised by Dire Hyaena and did not come into human contact until she was 12. At 12 she and one Hyaena companion were out hunting when she became injured and was luckily found by a group of escaped slaves (various races) out hunting who took her back to their camp in the jungle.
Here she learnt human ways and first picked up the Asegai (short spear), she also learnt to dance and soon combined these into her own fighting technique known as the War Dance (thus creating the Wardancer PrC which has special abilities like Evasion, Speed, and at high levels Blur and Abundant step).

Nafanua (Fighter/Wardancer) Nafanua is the daughter of an Abberation (which I based on a modified fiendish otyugh) and his human 'wife'. Nafanua became the leader of the rebellion against the Empire of the Tui-Tapua which was occupying her homeland. As the child of an Abberation she did have certain unusual abilities including regeneration and black tentacles.

Chu-Pa Chieh (Half-orc Barbarian/Rogue) A Rare being in the Golden Empire of Ti'en the half-orc Chu-Pa Chieh was a gluttonous and lacivious barbarian from the west who often met fear and hostility. Teaming up with a pair of humans (one a scorverer and the other a spearfighter) the three started a scam whereby Chu-Pa Chieh would sneak into a house and pretend to be a demon as he raided the storeroom. His two companions would then appear pretending to be demon hunters and in return for gold would 'exorcise' the demon. The real fun started when the three, whilst running a scam, met a real demon!
