The Deadliest Swordsman

The best swordsman would fight with a sword in one hand and a wooden shield in the other. He would wave the sword around to distract you and then kill you with the shield.

Spikes + shield charge + rhino hide +... +...

The frenzied berzerker above with this combo (and a potion of Assassin Sense just for fun) would be doing some serious damage, but not with the sword. A shield is a better weapon in both the general, and special, case.

I second Iaijutsu Masters. An 10th level Iaijutsu Master with maxed Iaijutsu Focus and a 20 Cha (Not hard to get at 20th Level) and power attack iaijutsu can be doing 1d10+10d6+50 In a single attack. Throw in expert tactician and one strike/two cuts and thats 4 attacks....in the suprise round. Did I mention he always gets a suprise round?...

It gets a whole lot nastier with two weapon fighting.

no ground is safe!

People seem to be forgetting about additional attacks that is available to the frenzied berserker when berserking. That would make a big difference. Stack that up with an extra attack from two levels of tribal defender and you can be doing major damage.

Half orc barbarian 1/fighter 7/tribal defender 2/frenzied berserker 10 with 25 strength.

When raging and berserking, 35 strength and attacks at +31/+31/+31/+26/+21/+16? for 2d6+14 each, without any magic, power attack or criticals. (I hope I did this right, don't have any books with me.)

Combined with Gigantic Weapon (netbook of feats) and Exotic Weapon Proficiency Heavy (Magic of Faerun) Flamberge (Quintessential Fighter) you can have a fighter swinging a huge heavy keen flamberge +5 of acidic burst and shocking burst for 6d6+35 damage per hit with a crit range on a keen weapon of 12-20 for double damage and 2d10 extra.

As for best prestige class, it depends. What AC is the opponent this swordsman is up against?
