:) True, but somebody (Anthony, Sean, Monte, can't remember) from WotC or someone WotC-related said that four attacks from your BAB was the cut-off limit even in the ELH.

Maybe it is there attempt at interjecting a bit of realism into the game? I mean, if you were a 60th-level fighter your attacks would be (just from your BAB):

+60/+55/+50/+45/+40/+35/+30/+25/+20/+15/+10/+5

Twelve attacks in six seconds.......cool, but maybe that's not what they wanted.
 

The way I see it, you would not be at +60 with your 60th-level fighter, since BAB and BSB will cease to work after 20th level the way they did before: +1 every other level for both (one at even, the other at odd levels), independant of class. That's the way it works for deities, and they said in that sidebar that those are rules taken from the ELH. So your 60th-Level Fighter will have +40/+35/+30/+25.

Personally, I think that the BAB and BSB should progress as it did, but without additional attacks. If you make them the same with every class, the classes become somewhat indistinct.

The reason why you stop at 4 attacks is that those additional attacks will slow down your game, and they won't help you much if you fight against something big: You have to consider that at epic levels, you can expect to fight against epic enemies, and a mere +5 or +10 BAB won't help you.

I think they have the fitha attack in the MM cause that's one of the core rulebooks. Nowhere in the core rules it's stated that you can't have more than 4 attacks solely by attack bonus - because, if you follow standard rules, you'll never have more than that as the hightest BAB you can have is +20/+15/+10/+5 (you can gain more with haste or a speed weapon, or with using off-hand weapons, however).
