I had a PC with a short sword +3, detect magic x3 daily, knock x1 daily, that when held functioned as peripiat of proof against poison. (1st ed) Before that, I had a paladin with a holy avenger +5, and a wizard with a staff of power. But I was younger then.

As a DM I gave out a long sword +2, +6 vs summoned creatures, that allowed pass without trace at will, animal friendship x3 daily, and polymorph self x1 daily. (1st ed)

Other than those, the minor artifacts in 'White Plume Mountain' come to mind. I'd have to go look at my notes to see what I've given out in 3rd edition. A few minor artifacts, but I don't remember the powers just at the momment. As a 3rd edition player, I don't know, a large shield +4? (I've not got alot of chance to be a player in 3rd.)

I've seen some insanely powerful items in other peoples campaigns. I generally didn't play in those long.

There was a sword in our 1st/2nd Ed. campaign called Carethian (sp?) (don't think it was related to the elven deity) which was an intelligent +660 longsword. However, each time you used a '+' to deal damage the power of the sword was reduced by that amount, and you had to power it up again by slaying things*. Also if you used more than +60 worth in one strike, the sword had a chance of breaking (it would deal the damage, then break).

*I think the power up was 1 HD slain equal to the next highest '+' for every restored '+'. So the darn thing took ages to power up, but could even scare demon princes and deities.

Not sure off the top of my head if that was THE most powerful item, but it was probably among the most powerful. ;)
 

As a character . . . nothing that special. Like gloves of dexterity +4 or something. Not very interesting.

As a GM, my players got to play around with an artifact that shows up now in War of the Burning Sky -- the Torch of the Burning Sky. It can call down firestorms and teleport armies. Nothing quite like using an artifact to take out the main villain with your army.

In 1E, my bard (yep, the PHB appendix, old-fashion, original PrC bard) had a sword that was modeled after Elric's Stormbringer. The stats were close if not right on the stats given for Stormbringer in the first printing Gods, Demigods and Heroes.

It was tough just keeping him alive with all the PCs trying to whack him and get the sword for themselves.

The nastiest items (beyond Stormbringer above and in a different campaign) we encountered were wielded by the dwarf janitors that cleaned up the local dungeon. They had +12 push brooms. We took a serious beating figuring out not to mess with the janitorial staff.

The Time Feather

It looked like the Mastermune from Chrono Cross

+25 acidic blast fiery blast icy blast lightning blast sonic blast keen vorpal everdancing two-bladed sword
- as a free action, it can turn into any other weapon (axes, whips, spears, etc...) or shield
- as an immediate action, can provide the bearer with immunity to any single energy type
- was sentient
- against deities, it could penetrate their damage reduction, always did critical damage, and on a critical hit could kill deities.

It was previously a sword known as the Avatar-Killer, but my player recovered it, reforged it into a weapon she could use, and became one of the deities' most feared beings in the Multiverse. She eventually ascended to become the head of the pantheon of deities :)

It had a host of other other spells that could be cast, but my player never used them and we kind of forgot about them.

My favourite epic game.
 

As a player, a +4 large shield with 9 charges of Resurrection in a 2e game.

As a DM, my PCs have some serious gear. Bracers of Armor +12 with +6 to Con, +12 equivalent weapons, book of infinite spells, Halaster's Horned Ring, Staff of the Archmagi, and a few other artifacts. But they're currently 25th level on average, having worked their way up, starting at 5th level in Undermountain. They've been in modules, or Undermountain itself the whole time, so all the gear they have is what has been written into the modules.
