I answered "none" but I missed Formian in the list. I used one of those not long ago when one of my players used Lesser Planar Ally. It was a taskmaster who helped him interrogate a captive. It used its dominate ability and when the iterrogation was over gave the captive a command to follow the PC's orders. They took great delight in making the captive pay for their hotel rooms, etc. :D

Gray render I've used in running a module and just faced one recently in a game. Rend with bite and tied to grapple check? Different rend from other monsters.

Lillend I thought I had summoned as a PC so I marked it above but I got them confused with the avoral so that's a false positive. I've got a module with a lillend but haven't used it yet.

Sphinx and Yeth Hounds I've used as a DM, both in 1e/2e games.

I've never seen a purple worm in a game despite their classic monster status.
 

The Razor Boar is one of those OGC from MMII.
I think they were created for Necromancer Games Creature Collection.

Where is the first poll?

Yep, from MMII and originally from one of the Creature Collections I believe.
 


Formian (Any) - yes and I once had a player create a modified character based on a worker.

Grick - yes, when I was throwing Cthulhu themes at the players

Purple Worm - yes, a dead one as an example of what the party could expect to find...they didn't though.

Shocker Lizard - yes, a friend and I had them as familiars...we were trying to get the entire party to be wizards with them as familiars.

Sphinx (any) - Yes, I was running an Egyptian themed game.

Chuul - no
Digester - yes, once
Formian (Any) - Oddly enough yes. I resent them as failed attempts to replace the Modrons, but I've used LN/LG ones in Arcadia, and evil expanionist versions swarming across cubes in Acheron. They work -really- well in Acheron.
Grey Render - no
Grick - once
Howler - Pandemonium isn't Pandemonium without them *evil grin*
Krenshar - no
Kraken - no
Lillend - Yes, on the Infinite Staircase, in Ysgard, and as a PC (who sadly was sawed in half during a plot arc in Pandemonium...)
Phantom Fungus - No. Is there some kind of nostalgia these guys have for folks? Seriously, looking at their picture in the MM, they're the stupidest looking things on the face of the planet. I'd feel shame if I used one in a game. My players would mock me.
Purple Worm - no
Ravid - Yes, on the quasielemental plane of Ash, paraelemental magma, and the ash/fire border regions. They're great, but they need to be in their environment to really make sense. Their 2e writeup in the PSMC III was awesome.
Razor Boar - no
Shield Guardian - I've had some NPC spellcasters with them, or versions of them
Shocker Lizard - As an NPC spellcaster's familiar
Sphinx (any) - no
Yeth Hound - Yes, as servitors of a lich, of some Night Hags, and once as an experimental subject by some yugoloths
Xill - Yes, frequently in the deep ethereal, and occasionally in the border ethereal. Again, like the Ravid, the 3e version doesn't convey the best about them from 2e. The PSMC III once again had some incredibly disturbing stuff on them.
 


The only ones I've never used/encountered are the digester, krenshar, and phantom fungus. These are three I really wish hadn't made the cut so that any of a number of superior creatures could have been core (especially yugoloths).

Krenshars are one of the biggest "joke creatures" among our group. For some reason, those face-peelin' cats just strike us as incredibly goofy.

The razor boar is one of my favorite creatures. Everytime we've used one they've been tough to defeat.
