Legend
Here's my list, along with a few notes:

Achaierai - Yes (ever since the 1e FF!)
Allip - Yes (first incorporeal monster I encountered as a player in 3e!)
Animated objects - No, surprisingly enough.
Aranea - Yes
Athach - No
Delver - No
Destrachan - Hell yeah! These guys are bad ass!
Devourer - Yes
Giant Squid - Yes
Girallon - Yes
Gorgon - Yes
Nightshade (any) - Yes
Phasm - No
Rast - No
Tarrasque - Yes, but I forgot to vote for them. Er, him, in most campaigns.
Thoqqua - Yes, love them ever since 1e.
Tojanida - No way. Tojanidas are the worst monster in 3e, imho.
Unicorn - Yes
Will o'wisp - Yes
Yrthak - Yes.

Legend
Achaierai - No big evil birds
Allip - No insane spooks used so far though I like their flavor
Animated objects - No I have never faced or used animated objects. Huh.
Aranea - Yes I've played one as a PC and encountered one as a bad guy as a pc. I remember reading about them in Isle of Dread.
Athach - No three armed freaks
Delver - No, just no
Destrachan - No sonics
Devourer - No I dislike devourers so much that I removed them and a beholder encounter from one module I was running and replaced them with a moon somethingorother (not the mooncalf, the other tentacled thing) from MMII.
Giant Squid - No. Sharks and octopi, yes.
Girallon - Yes. encountered as a PC in a module
Gorgon - Yes. Had a PC of mine charm a couple into being attack beasts
Nightshade (any) - No, though I'm running a module with them as a wandering encounter in a part the PCs haven't gotten to yet.
Phasm - yes, encountered as a PC in a module.
Rast - No.
Tarrasque - No
Thoqqua - No. I liked the 1e FF pic though.
Tojanida - No
Unicorn - Yes, encountered as a PC
Will o'wisp - Yes, encountered as a PC, I used them as an encounter/trap in 1e/2e.
Yrthak - No. sightless sonic birds? No.

Huh?

I've been playing since 1E days and I never ran across the thoqqua until 3E.

1e FF. Cool picture of a blazing molten fire thing blasting through a cavern wall. Looks slightly sci-fi.

Wow.

That is my favorite monster book ever and I never ever noticed it! I just went and looked, all skeptical of your ascertion. . . That means I used more 1E FF monsters in my last campaign than I thought I had!

For the record I used:

Cifal
Crypt Thing
Dakon (spelled Dakkon in my setting)
Elf, Drow
Fire Newt
Flail Snail (with elemental fire template)
Giant Strider
Hook Horror
Kuo Toa
Needleman
Protein Polymorph
Quaggoth
Slaadi, Blue
Thoqqua ;)

Allip - I've used these guys a few times

Animated objects - Lots of times, usually with a Ravid being involved.

Devourer - These guys rock. Soul devouring = creepy. They shouldn't be undead however. Somewhere along the line 'they look undead but specifically aren't' from 2e became 'they're undead' in 3.x. Bizarre.

Nightshade (any) - Yes, but I think they were only alluded to, or avoided, rather than being fought by the PCs.

Phasm - These things look like a Kandinsky painting. Too goofy.

Rast - Yes, but only on some of the inner planes where they were native.

Will o'wisp - I've used them in a lower level game, but only once.

Yrthak - I had some of these critters used as flying mounts by some Rakshasa in Acheron early in my 1st campaign.

What did you use for conversions? ToHR? MMII? Other sources? Your own conversions?

Cifal - My own conversion
Crypt Thing - My own conversion based on the version on Creature Catalog version
Dakon (spelled Dakkon in my setting) - My own conversion
Elf, Drow - My own conversion ("drow" is a template for elves in Aquerra)
Fire Newt - Monsters of Faerun
Flail Snail (with elemental fire template) - My own conversion
Giant Strider - Monsters of Faerun
Hook Horror - Hmm, I thought it was a Creature Catalog version, but can't find it in there now. Is it in MoF? If so, then there. If not, somewhere online.
Kuo Toa - MM
Needleman - My own conversion
Protein Polymorph - My own conversion
Quaggoth - MoF
Slaadi, Blue - MM
Thoqqua - MM

Autoexreginated
Of the above, I've used the Thoqqua (Summoned one recently to melt a passageway with my druid!), the Allip (as a DM), Animated objects (several different games), Delvers (bunch of Shadovar in FR using one to dig for a netherese artifact!), Destrachan (trapped a bunch of PCs in a cave-in with one), Girallon (encountered), Gorgon (used in Eberron), Tarrasque (PCs threatened with stopping one from waking!), Unicorns (used one from a distance, not met), and the Will o' Wisp (Never in 3E, but I have in 2E),

The Phasm, Rast, and Tojanida, like the Xorn, are just too funky, aberrant, and uninspired for me to use.

It could have been from the Creature Catalog. It went to the Crypt once it popped up in the MMII.
