Throwing things and creatures

How does one handle a creature picking up another creature and throwing them? E.g., a giant who flings a gnome over her shoulder. What about inanimate objects? How do I know how far a creature can throw something, and how heavy that sometihng (or someone) can be?

Apologies if there's an obvious spot in the rules that explain this.

There are actually some feats out there (not in a book I have, so I can't give details) which allow that, so it seems to not be something you can do without the specific feat.

One of the Rules of the Game articles on grappling at the Wizards website suggested using the following option (against an enemy you have pinned) as a house rule:

Toss Your Foe: Make an opposed grapple check as a melee attack. If you succeed, you can literally pick up your foe (provided you can lift your foe's weight). Make a Strength check; if your result is at least 10, you toss your foe 5 feet. For every 5 points your Strength check result exceeds 10, you toss your foe another 5 feet, to a maximum of 25 feet.

There is Fling Enemy and Fling Ally feats in Races of Stone. If you like, I can post it later.
