I think of the default D&D setting as Vance DMs Howard, Leiber, and Anderson as they attempt to loot Moria or Melnibone or wherever. If your homebrew is a bit too Tolkieny, pick another setting or two to steal from. A setting that is 30% Middle Earth, 30% Hyborea, 30% Discworld, and 10% homebrew will seem much less derivative that a setting that is 60% Middle Earth and 40% homebrew.
