Off the top add these changes to Alter Self, Polymorph, Polymorph Any Object, Shapechange, Baleful Polymorph, Animal Shapes, Alternate form ability, and Change shape ability.

Change +10 bonus to disguise to +10 Inherent bonus.

Add at the end:
"If this spell is used to assume a larger form it counts as a magical effect that increases size. If this spell is used to assume a smaller form it counts as a magical effects that reduces size. Neither effect stacks with similar effects." (removes doubling up of size changing effects similar to Enlarge/Reduce and RIghteous Might.

Alter Self changes:
last sentence, 1st paragraph:
"You can change into a member of your own kind or even into yourself regardless of the maximum HD." (provision for casters with more than 5 natural HD).

"You keep all extraordinary special attacks and qualities derived from class levels, but you lose any from your normal form that are not derived from class levels which are not duplicated by the assumed form." (provision for casters turning into their same race or having abilities shared by both)

"Physical qualities include ..., natural armor bonus (to a maximum of your HD cap), ..." (limit the Natural Armor granted by the spell)

After line about not getting more advantageous 2 hand attacks: "You may not effectively attack with more limbs or heads than possessed by your normal form. " (exclusion for Hydra form and other similar large numbers of attacks.)

"You do not gain any extraordinary special attacks or special qualities not noted above under physical qualities, such as ... unless they are properties of your normal form and the assumed form (such as when turning into a member of your own race)." (clarifying reference to keeping abilities when becoming same race or self)

Polymorph changes:
Change duration to: "1 min/level; see text"

Change the line about type to "Your creature type and subtype (if any) remain the same regardless of your new form." (counter to conbination with Animal growth, Awaken, and gaining additional abilities based on Type).

Rewrite the 3rd paragraph:
"The target gains a +4 size bonus to Strength, a –2 size penalty to Dexterity (to a minimum of 1) for each size category larger than normal; or gains a +2 size bonus to Dexterity, a –2 size penalty to Strength (to a minimum of 1) for each size category smaller than normal. It receives the size modifier to attacks and defense normal for a creature of its size. It retains the mental ability scores (Int, Wis, Cha) and Constitution of its original form.
It also gains all most extraordinary special attacks possessed by the form (subject to DM discretion) but does not gain the extraordinary special qualities possessed by the new form unless they are properties of your normal form and the assumed form (such as when turning into a member of your own race). The spell does not grant any supernatural or spell-like abilities.
" (remove stat replacement for size modifier which prevents stacking and excessive bonuses.)

Add at the end:
"This spell can also be used to duplicate the effects of enlarge person, reduce person, or alter self. When used to duplicate these spells its duration is increased to 1 hour/level." (add in the option of using Polymorph for longer term disguises or other longer effects. Slight buff to adjust for severe reduction in spell power)

Animal Growth:
Change modifiers closer to Righteous Might and Enlarge Person:
"grants it a +4 size bonus to Strength and a +2 size bonus to Constitution (and thus an extra 1 hit points per HD), "

"The spell also grants each subject damage reduction 5 /magic "
(Cuts the bonuses in half since they stat with other spell enhancements)

Polymorph Any Object"

Adjust duration downward to remove permanent forms:
"Duration Factor Duration Example
0 10 rounds Pebble to human
2 10 minutes Marionette to human
4 1 hour Human to marionette
5 4 hours Lizard to manticore
6 12 hours Sheep to wool coat
7 1 day Shrew to manticore
9+ 2 days Manticore to shrew"

"Unlike polymorph, polymorph any object does grant the creature the Constitution score of its new form. If the original form didn’t have a Wisdom, Intelligence, or Charisma score, it gains those scores as appropriate for the new form." (Bring in Constitution as an option)

"A nonmagical object cannot be made into a magic item with this spell. Magical properties of items aren’t affected by this spell." (Adds option to change form of magic items and allow magical resizing normal for the transformation spells.)

Shapechange:
"You gain most extraordinary and supernatural abilities (both attacks and qualities) of the assumed form, but you lose your own supernatural abilities (subject to DM discretion). " (add DM veto power for abilities granted by Shapechange outside the normal abilities grantable by spells)

I pretty much agreed with Rich Burlew's baker's dozen of problem/issues but feel like he splits the core spell into too many different spells.

I attempted to cover all the issues without completely breaking them out into new spells.

Adjust Alter Self since it impacts Polymorph and then just adjust Polymorph at it's core. After that Wildshape, and the other effects dependent on Poly are reduced without rewriting them.

Rich's stuff goes into too much specifics and doesn't meet one of my design parameters - can I remember the basic effects off the top of my head.

I also wanted to level the statistics gained for each form size so that there is less pronounced optimization when selecting forms (or at least optimized forms make less of a difference). Optimized criteria are size (balancing str/dex adjustment vs. size modifier - ends up a trade of AC for damage), number of attacks/base damage, natural armor (up to cap), special attacks (with a nod to DM veto), and movement (able to be duplicated by lower level spells for longer durations).

Change +10 bonus to disguise to +10 Inherent bonus.

Not sure what to call this bonus. Inherent doesn't seem right. The Eberron changling lists it as a Circumstance bonus but that is the same bonus as the MW Disguise kit which kind of implies that having the right tools won't help. I'd go competance but then having a magic item that increases disguise skill wouldn't stack (implying that higher skill wouldn't help). Maybe it needs a new type 'transformation' bonus to represent actually changing shape. What ever the name all the +10 transformation bonuses should have the same type and not stack.

Polymorph changes:
...

Realized this wasn't worded as I intended: Replace last sentence.
It retains the ability scores of its original form.

The only stats that are adjusted change by virtue of the size modifier to Str and Dex. The normal Str and Dex don't change.

"Unlike polymorph, polymorph any object does grant the creature the Constitution score of its new form. If the original form didn’t have a Wisdom, Intelligence, or Charisma score, it gains those scores as appropriate for the new form."

Rewrite this:
Unlike polymorph, polymorph any object can grant a creature or object a statistic score they don't have or remove a statistic that the new form doesn't have. The statistic gained or lost is that of an average creature or object of the form chosen.

Add to Shapechange:
Unlike polymorph, shapechange can grant a creature or object a physical statistic score they don't have or remove a physical statistic that the new form doesn't have. The statistic gained or lost is that of an average creature or object of the form chosen.
