I would like people to post examples of uber tanks. Here is an example I will think of more later...
Cleric of Lathander vs. undead. With Morninglord Prestige Class.

dwarf, fighter, full plate (or something even bigger), dwarven waraxe, shield; cha 6. Put no points into Cha or Dex, get PrC's like dwarven defender or stone warden (which will let you use con instead of cha for your AC). Maybe do fighter/barbarian.
 


There's two things you need to remember.

1. A tank needs to be able to survive.
2. A tank needs to either be mobile, OR have a way of bringing foes to him

Too many tanks ignore number 2. If the foes are not attacking the tank, the tank may as well pack up and go home - he's not doing his job.

The best approach I have found to #2 is to maintain a good offense. If you have an offense, you are a threat, and the foes think they have to deal with you somehow. If you have no offense, the foes can ignore you.

So, for example, my Dwarven Defender puts stat boosts into Strength (for offense) and does not use abilities like Expertise or equipment like Tower Shields. Expertise and Tower Shields might keep you alive, but they reduce your offensive capabilities.

The other part that helps with the Dwarven Defender Tank is if the rest of the party is ranged attackers. In that case, you can stand inbetween the ranged party members and the bad guys. If the bad guys want to melee the party casters, the bad guys have to go through or around the Dwarven Defender.
 


While they might lack the offensive punch that most full BAB classes can muster, a psychic warrior with Psionic Body and a host of other psionic feats that concentrates on movement and defensive powers, along with the judicious use of Vigor can tank it out with the best of them, while still maintaining enough offensive ability to make themselves into a worthwhile target. Such builds do require a tremendous amount of resource management.

Jesus, this guy- a pc in my epic campaign- can deal something like 700 hp on a charge when he pulls out all the stops and does the "Inoke Special".

Yeah Inoke is more of an offensive tank than a defensive one for sure. Link is in my sig.
