Underdark (Your Visions)

You could have a lot of fun with different terrain - including underground rivers, lakes (and islands).

You could have a large dam on a river, powering some wizard's experiments... or even Underdark empires, with multiple small kingdoms (dwarven or otherwise) and well-maintained highways leading to each area (and the dams could simply be flood control dams).

Another thing ... how about a race of flying creatures (there are some, I'm sure) fighting in formations?

Half-dragon stirges. They're just like regular stirges--but with breath weapons.

Got attacked by these at GenCon in the Living Greyhawk "special" (run only at GenCon, so not sure if I need the spoiler space, but better safe than sorry).

The DM later told us that several low level parties got TPK'd by that encounter, because the stirges had the breath weapon even at APL 2 (average party level = 2 is the lowest level of LG).

don't overlook the obvious, ie:
spiders and other insect like creatures
driders and such
medusa and gorgons
traps both set and natural like landslides
water - or the lack thereof
