Level Up (A5E) Voidrunner's codex - 'Always Prepared' Maneuver, is this a broken feature?

At least. I'd wager that with 2x the number of attacks per turn, combats will be shorter and PCs will take less damage, which means that Hit Dice could be used to regain exertion instead of Hit Points (this takes only 1 minute). In any case, even assuming the typical combat lasts the average 3 rounds, this means that this fighter could go nova every round of every combat for 3 typical fights every short rest, and just keep doing it.
 

I would probably go a different route with this and change the wording on the Ready Action to state that the trigger cannot be something directly caused by your own actions. That would change it so that you can only use the readied action out of your turn (at least as far as I can see), and have to take into account the other risks associated with it. That would immediately solve the problem with Extra attack, as well as Action Surge-esque spellcasting.
That also makes sense, although it intervenes on a different aspect (that still needs some ironing out, of course).
To be fair there may be rare instances when I want to react to something that is triggered by my own actions, like a counter-counter attack, but I guess those instances are both very rare, quite convoluted, and can probably better be dealt with using normal actions instead of setting up triggers etc.
An example could be intentionally triggering a trap and trying to deflect the trap attack towards an enemy.
 


If we ignore the effects of power creep that comes with new material how much spell abuse could you get away with using Always Prepared currently?
Not sure much harm can be done with that: spellcasting is always limited to 1 leveled spell per turn, and one must keep concentration on the ready spell.

That is only the case with bonus action spells, interestingly.

A spellcaster can’t cast a spell as a bonus action on the same turn that they cast another spell, unless the second spell is a cantrip with a casting time of one action.

As for concentration, you can just make the readied spell your concentration spell, and use non-concentration spells for the normal (or subsequent) actions.
