Well, in our games, we usually spend two or three weeks on character creation before a campaign begins.
Lisa, for example, will say, "Hey, guys, wanna play in a campaign? here's the setting, here's the starting level. I'll be starting the game in a couple of weeks, once I flesh out the first adventure."
And then we'll come up with elaborate backstories, lists of contacts, descriptions of equipment, journal entries, full-page illustrations of our characters, and the like.
Oh yeah--and a character sheet.
Given that level of preparation, death in our games is pretty rare. When it does occur, people tend to play a PC different from the last one they played.
Of course, we generally play a version of D&D that's much more combat-light than the standard game. We occasionally will go three exciting sessions without rolling initiative once. For a game with a normal amount of killin, our method won't really apply.
