As a result of a discussion over at RPG.NET, here's an attempt at adding the missing "forced march" rule and at the same time making all kinds of hardships use the same rule. Please discuss.
Being out of air, water, food, sleep all means you're deprived. A forced march (walking more than the ten hours allowed by the PHB) also mean you're deprived. Of rest.
And we'll handle all of these cases as one and the same standard 4E disease.
The stages will be something like this:
Cured: You're free from your deprivation. Normally you can't reach this stage without getting access to whatever it is you lack: air, drink, food, rest, sleep.
Recovering: No penalties. This is the best stage you can hope for without actually being cured.
Infection: You lose a healing surge each time you fail that can't be regained until you're free of the Deprivation.
Second Stage: You don't regain any healing surges during your next extended rest. (Yes, this means each and every extended rest until you improve!)
Final Stage: You're considered out of healing surges for healing purposes (you only regain the 1 hp from a surge). You can still activate surge-fueled powers normally if you have any left.
The only difference between being out of air, out of food etc. is how soon you'll have to start making Endurance checks.
Air: Three minutes (as per standard rules).
Drink: Three days (as per standard rules).
Food: Three weeks (as per standard rules).
Rest: Ten hours (as per standard rules).
Sleep: 24 hours (as per standard rules - somewhat, at least).
And how often you need to repeat those Endurance checks. Here are my suggestions:
Air: Each round.
Drink: Each encounter or every 12 hours.
Food: Each day.
Rest: Each encounter or every three hours.
Sleep: Each day.
